4Noah utilities by William Pilgrim can help make previously inaccessible PC games more accessible. They can help to reduce the number of controls you need, give you automatic firing and more.

Have a look at some of William's videos here: http://www.youtube.com/user/mojofltr that explains more (most are using a Nintendo NES emulator for the demo games).

Just recently I've been using 4Noah alongside a Playstation emulator to make some driving games and golf games playable with just one or two switches. It can all take a bit of getting your head around, but it's sweet when you get it all working. Especially when you make the previously inaccessible, accessible.
